Overview

Enrollment begins for Fall 2010 in November of 2009. All interested applicants must fill out an application as detailed below for a spot in Kindergarten to 5th grade.

You’ll need the following documents:

  • A completed application form (Donwload English Version) Or (Descarga en Español)
  • Your child’s original birth certificate (or passport)
  • An original proof of residence (current utility bill, mortgage or lease papers, etc.)
  • Immunization records
    • 4 DPT (one must be after 4th birthday)
    • 3 Oral Polio
    • 2 MMR (both must be after 1st birthday)
    • 2 Varicella (both must be after 1st birthday)
  • Your child’s original Social Security card (if child has one)
